Welcome to the online UMass Chan-Shriver Center LEND Advanced Leadership Fellowship Program application system.

The LEND program provides graduate-level interdisciplinary training to improve the health of children and youth with developmental disabilities. The Advanced Leadership Fellowship Program is a nine-month intensive program that trains Fellows to radically rethink the ways in which health care, services, and supports are delivered to children and youth and their families, and to develop the knowledge, skills, and tools to make policy recommendations, support exemplary services, devise new training strategies, and lead systems change efforts. 

In order to apply, you must have an account with SurveyMonkey Apply. If you do not have an account, you can create one after selecting "Apply" by selecting the "Register" option on the page that follows.
